Entry Form
Online entry is the preferred method. However if you prefer to use more manual methods, please print out the blank one on this page. You may need to set the text size on your browser to Medium or below. If you don't have a printer, then a 'homemade' version is quite acceptable as long as it contains all the information!
Final instructions and results - will be on the web site for you to read or print out. Contact the Entries Secretary if this presents a problem for you for the Final Instructions.
Note that age is as at 1st January, 2009 and not on the day of the event. And you must be 18 or over to take part (under 18s see NATO web site).
If you already have your own SI card, then enter the number in the appropriate box. If you need to hire one, then please write HIRE instead. Pairs competitors - you only need one SI card per pair. Please include the fee for the hire card with your entry. The hire charge is 50p per day (£1 for both days). Note that lost or non returned cards will be charged at £30 per SI card (hopefully there won't be any!!).
Only one entry form is necessary per entry for the Pairs classes, not one for each member of the pair.
Closing date - entry forms to arrive by Monday 10th August 2009. Late entry fee applies after this date.
